Are there any forms or documents needed from a patients nee?
New patient, we require id, and medical aid to open a file. For cash-paying patients we require cash and id.
File details are filled by the receptionists.
What insurances do you accept?
We accept cash and medical aid patients.
BHF Registered (for medical billing).
Registered with HPCSA,
Dispensing license (Department of Health)
MPS member (indemnity). Which is an insurance of company for medical doctors, in cases of lawsuits.
